Passive Range-of-Motion Exercises for Dogs: A Vet-Guided How-To

Passive range-of-motion work is one of the few joint interventions an owner can do correctly at home in under ten minutes, and most owners are never shown how.

Sixty percent of the dog’s frame is still under a blanket at 7 a.m., and the owner is on the kitchen floor with a fourteen-year-old dachshund named Biscuit, one hand cupped under the stifle, the other at the hock, moving the leg through a slow arc she was shown three days earlier after a fibrocartilaginous embolism left the hind end weak. That is roughly how most owners first encounter passive range-of-motion (PROM) work: not as a wellness routine, but as an assignment handed to them at a discharge appointment, with maybe two minutes of demonstration and a lot riding on getting it right.

What should you actually do? Lay the dog on a padded, non-slip surface in lateral recumbency. Support the limb above and below the joint you’re working — one hand stabilizes the femur or humerus, the other guides the paw. Move the joint slowly into flexion, hold briefly at the first point of resistance, then return to a neutral, relaxed position. Repeat through extension. Never push past resistance, and stop immediately if the dog tenses, vocalizes, or pulls away.

What PROM is actually doing to the joint

PROM is movement without active muscle contraction — you’re the engine, not the dog. According to the Washington State University Veterinary Teaching Hospital, these exercises are designed to maintain and increase joint mobility through gentle stretches of the surrounding muscles and tendons, and they’re used in dogs that are arthritic, recovering from surgery, or dealing with muscle atrophy from disuse.

The mechanism is mostly mechanical, not medicinal. Moving a joint through its arc circulates synovial fluid, which is the medium that carries nutrients to cartilage and clears out some of the inflammatory byproducts that accumulate when a limb sits still. A recent prospective study published in Frontiers in Veterinary Science looked at a structured therapeutic exercise program’s effect on range of motion and muscle mass in dogs with osteoarthritis, reinforcing that deliberate, repeated joint movement is a meaningful lever in managing chronic joint disease — not just a box to check after surgery.

For a postoperative stifle, a common protocol looks like three sets of ten flexion repetitions and three sets of ten extension repetitions, performed a few times daily, each hold lasting only a second or two. For an arthritic senior who isn’t post-op, the frequency is usually lower and the goal shifts from preventing contracture to simply keeping the joint from stiffening further.

Where I’ve watched this go right, and where it doesn’t

In my practice, I’ve worked with a twelve-year-old Cavalier King Charles Spaniel, eleven pounds, whose owner noticed a subtle hitch in the right hind leg after a slip on hardwood floors — nothing dramatic, just a reluctance to fully extend when trotting across the living room in the evenings. We started twice-daily PROM on the stifle and hip, five minutes each session, done on a bath mat in the hallway because that was the only non-slip surface in the apartment. The owner tracked one thing: how quickly the dog would follow her up a single interior step after resting. At week one it was a hesitant scramble; by week four it was a normal hop. I want to be careful here — that’s one dog’s trajectory, not a study result, and it doesn’t tell you what would have happened without the exercises. But it’s the kind of case that convinces an owner the ten minutes matter, and it’s why I keep prescribing it.

Where this goes wrong is almost always technique, not intent. Owners who weren’t shown the joint’s actual anatomical limits sometimes extend a hip or stifle well past where the dog is comfortable, mistaking a dog’s stoic tolerance for consent. Where supplement ingredients fit, and where they stop

PROM addresses mechanics — fluid movement, soft tissue elasticity, contracture prevention. It doesn’t address the biochemistry of joint inflammation or cartilage turnover, which is where nutraceutical ingredients are studied separately. Chondroitin has evidence supporting healthy synovial fluid homeostasis, per a peer-reviewed study, and glucosamine HCl has been shown to prevent experimentally induced cartilage degradation in vitro, per a review in the Journal of Veterinary Medical Science. Green-lipped mussel powder has in vitro and rodent-model evidence — compounds derived from it alleviated paw swelling and reduced inflammatory markers including TNF-α and IL-17 in a study published in Nutrients.

I’ll be direct about the limit of that evidence: most of it is in vitro or drawn from other species’ inflammatory models, not large-scale canine clinical trials measuring lameness scores. Glucosamine and chondroitin are among the most studied joint support ingredients for dogs, but “most studied” is not the same as “definitively proven” in every formulation and dose. Structure/function language — “may help support joint flexibility” — is the honest framing, and it’s a complement to mechanical work like PROM, not a substitute for it. A dog getting a daily joint supplement still needs the joint moved if it’s stiffening from disuse.

Building it into a week that also includes real movement

PROM is not a replacement for appropriate weight-bearing exercise once your vet clears it. If your dog has been sent home with PROM instructions, the priority is technique fidelity over frequency — a correctly performed session twice a day beats four sloppy ones. If your dog is arthritic and hasn’t had surgery, ask your vet whether PROM is appropriate at all; some presentations do better with active, weight-bearing movement instead. Until then, the mechanical case for moving a stiff joint gently and often remains one of the more defensible, low-risk things an owner can do at home.
